Contact Us by Phone at 800-231-8312 - We are here to help and represent you. We are your point of contact. We will ask you for the facts of the loss and details on your damages, at which time we will submit a claim form to the insurance carrier.

Contact the Insurance Company by phone - If desired, you can call the insurance company directly. Go to SMH website, click service center tab and direct claims, locate your carrier and call the phone number listed. You will be asked to provide information from your insurance policy, facts of the loss and damages.

Submit a Claim Through the Web - As technology continues to improve; more carriers are taking claims through their website. If this is how you prefer to submit a claim, go to SMH website, click service center tab and direct claims, locate your carrier and select the link click here to file a claim. Once you are on your carriers website, click on the "Report a Claim" link. You will be asked to provide information from your insurance policy, facts of the loss and damages.

When you are reporting a automobile claim, be sure to have as much of the following information on hand as possible:

• Date, time and location of the accident.
• The current location of your vehicle and whether it is drivable.
• Information on the other driver, including name, address, phone number and driver license number.
• Details on the other vehicles involved, including insurance carrier, policy number, make and model of vehicle, license plate and VIN. You can get this information from the individuals automobile insurance ID card they should have in their glove box.
• Contact information for any witnesses and law enforcement agencies that arrived at the scene.
• Any injuries reported at the scene.

Once the claim is submitted and assigned to a claim adjustor, you will be contacted by that adjustor who will walk you through the claims process.
